Application Packet - An Online Application Packet is available here.  Please print out the application, complete it, and mail or fax it to the Vision of Hope office.  The office mailing address is Vision of Hope Attn:  Intake Committee 5652 Mercy Way Lafayette, IN  4790.  Completed applications may also be faxed to (765)449-3703.
